Bo’ness Car 4U will hold, your name, postal address, e-mail address, telephone number, Patient ID, Medical Practice and details of the location, times and dates of your hospital appointments on our secure database. Medical data is not held by us.
This database is only used to allow us to provide transport services to you. We will hold your details for five years in a secure, password-protected file on a password-protected computer. We will not pass your information to third parties.

By requesting our hospital transport service, you agree that your medical practice can provide us with your name and phone number.
